Care Coordination at TidalHealth
BERLIN, Md. – In this week’s Health Beat, brought to you by TidalHealth, WMDT spoke with Nicole Amores, a full-time case manager with TidalHealth Atlantic.
Amores spoke with us about how care coordination has improved at TidalHealth Atlantic, formerly known as Atlantic General Hospital, since merging with the larger health conglomerate.
“Care coordination has improved greatly since we’ve merged together,” she said. “Having one medical record makes the transition from inpatient to outpatient almost seamless. Assessment is the key to care coordination; we have to know the needs and know what’s available in our community to meet those needs.”
Amores says assessment is key to identifying resources for patients and formulating a care plan — all of which can be accessed in MyChart.
“So when you leave Atlantic General and you go into your next day, follow-up to see your primary care provider, they’re going to have the same information, the same protocols,” Amores said. “They’re going to see all the same information that we have just treated you, discharged you with your plan of care. They will pick up that plan of care when they open that chart and continue that plan of care.”
Amores says it is the responsibility of care coordinators and case managers to ensure patients don’t slip through the cracks. She encourages all patients to set up MyChart as a way to ensure that doesn’t happen.
