PRMC introducing Care Transitions Program
SALISBURY, Md. – Peninsula Regional Medical Center has announced that they will launch a new program later this month that will contact discharged patients to check on their recovery process.
The Care Transitions program, scheduled to launch on January 21st, is a phone call service that contacts all inpatients and observation patients after their discharge. Patients will receive an automated call that asks them a few questions, including how their health is in comparison to the day before, if they have questions about their medications, whether or not they have all the information necessary to schedule a follow up appointment, if they were satisfied with the care they received, and more.
Officials from the hospital say that any negative response or a response indicating any obstruction to care will be flagged immediately and a specially trained PRMC nurse will contact the person to obtain more information about their concerns. They will then be forwarded to the appropriate healthcare professional for quick resolution.
All calls will be documented, which will allow hospital staff to refine their discharge process if they notice any trends. This data will also give clinical leaders the chance to make more real time process improvements.
This program will not replace PRMC’s patient experience survey process, but will complement it and help address small healthcare issues before they become larger problems that impact care and recovery. PRMC is asking all patients to be honest and share any issues they feel are affecting their recovery in a negative way.
The program will be expanded on February 25th, when all patients discharged from the Emergency/Trauma Center will receive a similar call to check on the status of their recovery.
